Every incident and near miss gets reported and investigated — without exception, and without blame as the first response. Root cause analysis drives corrective action, and lessons learned are shared across the entire fleet.

STL's incident reporting policy requires that all incidents, regardless of severity, and all near misses, are documented within 24 hours. Root cause investigation goes beyond the immediate cause to identify systemic factors — equipment, process, training, or environmental conditions that contributed.

Corrective actions are tracked to completion, not just recommended. When an incident reveals a gap that could affect other drivers or routes, that learning is shared across the fleet immediately — through toolbox talks, policy bulletins, or direct driver communication. We treat near misses with the same seriousness as actual incidents, because the only difference is luck.
